Creating Incidents in HP Service Manager
To create incidents in HP Service Manager many small miracles need to occur, or you can use opService to simplify the process.
Static Configuration Items
A single NMIS server will be opening Incidents for many nodes, but may be configured differently to other NMIS servers in the master/poller cluster, to support this, a set of properties need to be configured per NMIS server for opService to work.

Tell NMIS to Start Sending JSON Events
NMIS support JSON event logging, this is what opService uses to integrate with NMIS. To enable this add an escalation policy with "json:server" as the target, this will start logging events which opService will process.
